What is the most serious error in phlebotomy?

The most serious error is failure to properly identify the patient. Even if everything else is done perfectly, the final result will not apply to the patient incorrectly presumed to be the source.

What is the number one cause of phlebotomy errors?

Wrong Patient Identification

Use of unlabelled tube or wrongly labeled tubes for collection of bloods. Some hospitals try to make use of pre-labeled tubes to avoid the work at the time of collection of samples. However this is one of the most frequent sources of error.

What is the most common complication in phlebotomy?

Hematoma: The most common complication of phlebotomy procedure.

What are the most common error in specimen collection?

Two of the most common errors that occur during specimen collection and handling are clotting and inaccurate volume. Exposure to temperature extremes may also cause specimens to be rejected for testing. Clotting compromises the integrity of a specimen, making it unsatisfactory for testing.

What are the four most common problems that occur with a venipuncture?

Serious complications were defined as cellulitis, phlebitis, diaphoresis, hypotension, near syncope, syncope, and seizure activity. Results: Minor bruising and hematoma were fairly common, involving 12.3% of venipunctures, with minor bruising being the most common reaction.

What are the 3 complications encountered in venipuncture?

How can you avoid venipuncture errors?

It can be avoided by checking the identity document of the patient and by always asking him to spell his name aloud. Use of electronic cards that effectively bar code the patient can do wonders to avoid such issues. Use of unlabelled tube or wrongly labeled tubes for collection of bloods.

Which is the most common vein used for venipuncture?

Venipuncture. The median cubital vein in the antecubital fossa is the most commonly used site due to its accessibility and size, followed by the neighboring cephalic and basilic veins [13,49,51,52].

How do you remember the order of draw?

The order of draw is an order of tubes phlebotomist should follow to prevent testing complications and cross-contamination of additives. The order of draw is Yellow, Light Blue, Red, Light Green, Dark Green, Lavender, and Gray. Way to remember this is by the saying Your brother Robert gives lovely gifts.

What causes petechiae during phlebotomy?

Petechiae occur when tiny blood vessels called capillaries break open. When this happens, blood leaks into the skin.

What are the common causes of error in laboratory testing?

The two major causes of analytical errors are instrument malfunction and operator error. Some examples of analytical errors include equipment malfunction, procedures (i.e., standard operating procedures and assay instructions) not followed, undetected failure of quality control, sample mixups, and test interference.

What is the most common error in the pre-analytical phase of lab testing?

The most commonly reported types of pre-analytical error are: a) missing sample and/or test request, b) wrong or missing identification, c) contamination from infusion route, d) haemolysed, clotted, and insufficient samples, e) inappropriate containers, f) inappropriate blood to anticoagulant ratio, and g) ...

What are the three categories of laboratory error?

Three general types of errors occur in lab measurements: random error, systematic error, and gross errors. Random (or indeterminate) errors are caused by uncontrollable fluctuations in variables that affect experimental results.

What are the sources of error during blood collection?

Common errors in blood sample collection include in- correct identification, wrong tube, insufficient sample quantity, clot- ting, hemolysis, and contamination. Unlike some other errors, mislabel- ing of blood samples is 100% pre- ventable.

What are the 3 main veins to draw blood?

The most site for venipuncture is the antecubital fossa located in the anterior elbow at the fold. This area houses three veins: the cephalic, median cubital, and basilic veins (Figure 1).

Which vein is used for blood collection?

Of these, the median cubital vein is usually the vein of choice for phlebotomy: It is typically more stable (less likely to roll), it lies more superficially, and the skin overlying it is less sensitive than the skin overlying the other veins. Antecubital veins, right arm.
