How often is CA125 wrong?
Daniel Davis The lack of a reliable test.
Conversely, not all ovarian cancer types cause elevations in CA125 and we estimate that 10% to 20% of all ovarian cancer escape this test (false negative test). Transvaginal ultrasound was promoted to diagnose ovarian cancer for many years. Can a CA125 test be wrong?
A high level of CA-125 can be a sign of cancer, particularly ovarian cancer. However, the test can have both false positive and false negative findings. CA-125 levels can be normal even if a person has cancer, or they can be elevated in a patient who does not have cancer.
How accurate are CA125 tests?
Results: Using a CA125 cutoff of 30 U/mL, an overall sensitivity of 94.8% (96.6% specificity) was obtained when comparing malignancies versus healthy postmenopausal controls, whereas a cutoff of 65 U/mL provided a sensitivity of 83.9% (99.6% specificity).
What is the false positive rate of CA125?
The "false positive" rate for this group of women is high. In fact, only about 3 out of 100 healthy women with elevated CA-125 actually have ovarian cancer. Any woman with an abnormal CA-125 test will need further tests, and sometimes invasive surgical procedures, to confirm the result.
Can you have a high CA125 and not have cancer?
If your CA 125 level is higher than expected, you may have a condition that isn't cancerous, or the test result could mean that you have ovarian, endometrial, peritoneal or fallopian tube cancer. Your provider may recommend other tests and procedures to determine your diagnosis.
Dr. William McGuire: What Do I Need To Do If My CA125 Is Rising?
Is it normal for CA-125 to fluctuate?
So, CA 125 values can and do fluctuate. Major treatment decisions, such as changing or discontinuing treatment, depend on multiple factors that you and your physician will consider. The trend in your CA 125 values is only one of these factors.
Can stress raise CA-125 levels?
There is also some evidence to suggest that stress may cause your body to generate increased CA-125. If you're experiencing any of the above health conditions, including high levels of stress, tell your doctor before having your test done.
Can tumor markers be wrong?
There's a chance that a tumor marker test can give a “false positive.” That means the results suggest a person has cancer or that the cancer is growing, even when it's not. A tumor marker can also give a “false negative,” which means the results suggest a person doesn't have cancer when they actually do.
Can ovarian cysts cause elevated CA-125?
Elevated CA-125 (a protein in the blood that's associated with ovarian cancer) is most often caused by common, ordinary or benign conditions such as uterine fibroids, ovarian cysts, endometriosis — or even just having your period.
Can stress cause tumor markers to rise?
A 2019 study, for example, showed that stress hormones can increase the number of pro-tumor immune cells in tumors. That could mean that stress not only wakes up dormant tumor cells but also provides the right environment for them to grow, Dr. Hildesheim explained. “It's the worst of both worlds,” he said.
Is CA125 a tumor marker?
Cancer antigen 125 (CA 125) is the only tumor marker recommended for clinical use in the diagnosis and management of ovarian cancer. The reference range of CA 125 is 0-35 units/mL (0-35 kU/L).
What level of CA125 indicates endometriosis?
The mean serum CA-125 level of women with endometriosis was 49.93±4.30 U/mL (range, 2-191 U/mL) in this study. Amara and colleagues found that CA-125 serum levels were higher in endometriosis patients when compared to control group during both periods of menstrual cycle (13).
What cancers does a CA125 detect?
Cancer antigen 125 (CA125) is a protein found on most ovarian cancer cells that is secreted into the blood stream and can be measured. CA125 can also be found on other normal and cancerous cells in the body.

Can a transvaginal ultrasound miss a tumor?
Transvaginal ultrasounds may be used to initially investigate symptoms, but they're only about 75 percent effective in detecting ovarian cancer.
How much do fibroids elevate CA-125?
An increase in CA125 levels was found in 20% of patients with leiomyoma. The mean CA125 level of the patients was 27.3 ± 38.1 U/mL. They found that the factors related to higher levels of CA125 in patients with leiomyoma were larger size (≥ 5 cm), subserosal location, and co-existence with adenomyosis.
Can polyps cause raised CA-125?
The levels of CA-125 in serum, and cervical and vaginal secretions were significantly increased in complex hyperplasia and endometrial cancer, but not in polyps and simple hyperplasia. There were significantly increased endometrial CA-125 expression in endometrial polyps, complex hyperplasia, and endometrial carcinoma.
What does slightly elevated CA-125 mean?
This test measures the amount of a protein called CA-125 (cancer antigen 125) in the blood. CA-125 levels are high in many women with ovarian cancer.
How can you tell if an ovarian cyst is cancerous?
Oftentimes imaging tests like ultrasound or MRI can determine if an ovarian cyst or tumor is benign or malignant. They may also want to test your blood for CA-125, a tumor marker, or preform a biopsy if there is any question. High levels of CA-125 may indicate the presence of ovarian cancer.
What is an ideal tumor marker?
The three most important characteristics of an ideal tumor marker are (a) it should be highly specific to a given tumor type, (b) it should provide a lead-time over clinical diagnosis and (c) it should be highly sensitive to avoid false positive results.
Why would tumor markers go up?
For example, noncancerous conditions can sometimes cause the levels of certain tumor markers to increase. In addition, not everyone with a particular type of cancer will have a higher level of a tumor marker associated with that cancer.
How can I lower my CA-125?
However, there are some lifestyle factors you can control to decrease your risk of ovarian cancer.
- Exercise and Diet. By maintaining a weekly exercise regime and a healthy diet, the risk of ovarian cancer decreases. ...
- Oral Contraceptives. ...
- Avoiding Carcinogens. ...

Does CA125 rise during period?
CA 125 levels were significantly elevated during menstruation in both groups. In women with anovulatory cycles, but not in those with ovulatory cycles, CA 125 levels were already increased in the premenstrual phase.
How much does CA125 increase with period?
Results: The mean CA125 concentrations of healthy women during menstruation and during the rest of the menstrual cycle were 12.2 and 10 U ml(-1), respectively. In this group, the mean CA125 concentration was an average of 22% higher during menstruation than during the rest of the menstrual cycle (P
